Group Adult Foster Care

Group Adult Foster Care (GAFC) is a Medicaid-Funded long-term care community option that provides elderly and/or disabled clients assistance with Activities of Daily Living (ADL’s) & Instruments of Activities of Daily Living (IADL’s), as follows:

  • Nurse Monitoring
  • Medication reminders
  • Prescription pickup/drop off
  • Assistance with Activities of Daily Living (ADL’s)
  • Light House Cleaning
  • Laundry
  • Meal Preparation
  • Case Management
  • Services 7 days a week

Eligibility Requirements

  • Adult 22 years and older
  • Individuals who require assistance with Activities of Daily Living (ADL’s) & Instruments of Activities of Daily Living (IADL’s)
  • Resides in the community in public housing that participates in GAFC
  • Member of MassHealth Standard, BMC HP, CCA, SWH and Tufts HP and other approved providers
    Physician approval
