Our services are free to participants enrolled in MassHealth Standard, BMC HP, SWH, CCA, Tufts HP and other approved providers

Eligibility Requirements

Adult Foster Care (AFC)

16 years of age and older
Individuals who require assistance with activities of daily living (ADL’s) & Instruments of Activities of Daily Living (IADL’s) and unable to live alone
Requires direct care
Member of MassHealth Standard, BMC HP, CCA, SWH and Tufts HP and other approved providers
Physician approval

Group Adult Foster Care (GAFC)

Adults 22 years and older
Individuals who require daily assistance with multiple personal care tasks
Resides in housing that participates in GAFC
Member of MassHealth
Physician approval
