Personal Care Home Maker, Home Health Aide and Certified Nursing Aide

Educational and Experience Required:

Certificate in Personal Care Home Maker, Home Health Aide and Certified Nursing Aide. Possess sensitivity to individual participants needs.

Summary:

A PCHM, HHA, CNA is recognized as a responsible individual who has been trained to provide personal care and those supportive services which are necessary to maintain a disabled or dependent person safety and comfort within a home setting.

Responsibilities and Duties:

1.   The PCHM, HHA, CNA gives or assist with personal care services such as:

  • Bathing – bed, tub, sponge of shower
  • Care of hair – combining, brushing, shampooing
  • Care of toe and fingernails: foot wash and soak, filing and cleaning only
  • Care of skin
  • Shaving with electrical razor only
  • Care of mouth, teeth and dentures
  • Helping client to bathroom or commode or bed pan
  • Dressing and undressing
  • Feeding (authorized on case basis)
  • Toileting
  • Ambulating per Care Plan and Nurse
  • Practices essential principles of universal precautions and infections control

2.   Performs household services such as:

  • Planning, preparing and serving meals
  • Purchasing food and household supplies
  • Making and changing beds
  • Cleaning and dusting household supplies
  • Mopping floor and vacuuming
  • Cleaning bathroom and kitchen
  • Washing dishes
  • Laundering and ironing

3.   Observes, records and reports to RN or Case Manager physical, mental or behavior changes in the client.

  • Any changes in family situation that may affect the client
  • Any emergency situations
  • Request from client, client’s family or physician for changes in service plan or schedule
