Adult Foster Care
Adult Foster Care Program provides assistance to individuals to either live with a caregiver or a caregiver can live with them, the caregiver will be trained by our staff and paid a stipend to provide daily care within a family setting rather than a nursing home or nursing facility.
Caregivers may be non-family members, or family members (except legally responsible relatives or spouses). The caregiver provides assistance with Activities of Daily Living (ADL’s) & Instruments of Activities of Daily Living (IADL’s) and 24-hour supervision.
Group Adult Foster Care
Group Adult Foster Care (GAFC) is a Medicaid-Funded (MassHealth) long-term care community option that provides elderly and/or disabled clients assistance with Activities of Daily Living (ADL’s) and Instruments of Activities of Daily Living (IADL’s). Total Care provides a multidisciplinary team that consist of a Registered Nurse, Case Manager and a Personal Care Home Maker (PCHM) or Home Health Aide (HHA) or Certified Nursing Assistant (CNA).
Private Pay
Private Pay Services (PPS) is a Private Pay-Funded for short-term and long-term care that provides clients of all ages assistance with Activities of Daily Living (ADL’s) and Instruments of Activities of Daily Living (IADL’s). Total Care can provide a multidisciplinary team that consist of a Registered Nurse, Case Manager and a Personal Care Home Maker (PCHM) or Home Health Aide (HHA) or Certified Nursing Assistant (CNA).
