Group Adult Foster Care
Group Adult Foster Care (GAFC) is a Medicaid-Funded long-term care community option that provides elderly and/or disabled clients assistance with Activities of Daily Living (ADL’s) & Instruments of Activities of Daily Living (IADL’s), as follows:
- Nurse Monitoring
- Medication reminders
- Prescription pickup/drop off
- Assistance with Activities of Daily Living (ADL’s)
- Light House Cleaning
- Laundry
- Meal Preparation
- Case Management
- Services 7 days a week
Eligibility Requirements
- Adult 22 years and older
- Individuals who require assistance with Activities of Daily Living (ADL’s) & Instruments of Activities of Daily Living (IADL’s)
- Resides in the community in public housing that participates in GAFC
- Member of MassHealth Standard, BMC HP, CCA, SWH and Tufts HP and other approved providers
